It’s my goal to make inclusive, HAES®-aligned nutrition therapy available to as many people as possible. To support that mission, I accept multiple insurance carriers and offer a sliding scale out of pocket fee structure.

However,

if your health insurance plan is “fully-funded” (NOT “self-funded”),
and has an effective date that is NOT before 03/23/2010 (NOT “grandfathered”),
and you meet the qualifying criteria (see below),
then you are entitled to certain FREE preventative care benefits, thanks to the Affordable Care Act
–> no co-pay, no co-insurance, no nada.

“intensive behavioral counseling interventions to promote a healthful diet and physical activity”
(aka medical nutrition therapy)
• Elevated blood sugar (>100 mg/dL fasting blood sugar, >5.7% A1c)

or

• “Obese(BMI ≥ 30)

or

• “Overweight(BMI ≥ 25)

and 1 of the following:
  • Elevated blood sugar (>100 mg/dL fasting blood sugar / >5.7% A1c)
  • Elevated blood pressure (>120 mmHg / >80 mmHg)
  • Elevated total cholesterol (>200 mg/dL)
  • Elevated LDL cholesterol (>130 mg/dL)
  • Elevated triglycerides (>150 mg/dL)
  • Low HDL cholesterol (<60 mg/dL)
  • Family history of:
    – Diabetes
    – Stroke
    – Heart disease
  • “Metabolic syndrome” – defined as having 3 of the following:
    – Elevated waist circumference (>35-40in)
    – Elevated triglycerides (>150 mg/dL)
    – Elevated blood pressure (>120 mmHg / >80 mmHg)
    – Elevated blood sugar (>100 mg/dL fasting / >5.7% A1c)
    – Low HDL cholesterol (<60 mg/dL)
